This packet is designed to help you navigate the various aspects of dTMS prior to scheduling your consultation, and should answer any additional questions related to pursuing dTMS treatment with us.
Please note: In order to schedule a consultation, you must contact us to complete a New Patient Screening over the phone to help determine if TMS is a safe and appropriate treatment option for you. We may further request records from your primary psychiatric provider.
Active/Returning Patients: Established and returning patients may click on the following link to our Patient Portal. Here you can check your next few scheduled appointments, complete any scheduled measures/scales we have assigned to you, and view your billing statement.
